Practical notes on interface and defect management from the field.

Why interfaces fail at the boundary

Most interface issues are not technical. They exist because nobody owns the boundary. A good register names a responsible party on each side, a due date and the evidence needed to close the item, and is reviewed at a regular board where open points become decisions.

Run the liability period as a process, not an inbox

When defects arrive from many sources, the risk is that they are handled in the order they arrive. Classifying by safety and operational impact, agreeing owners across suppliers and reporting ageing items weekly keeps closure predictable and the evidence complete.

From open point to signed agreement

An open interface point should end in one of three ways: an action with an owner, an escalation, or a signed agreement. Writing the agreement early, even in draft, makes the disagreement concrete and shortens the path to resolution.
